OpenEyes is a collaborative, open source software development project led by Moorfields Eye Hospital. The initial focus of the project is on Ophthalmology, but the design is sufficiently flexible to eventually be used for any clinical specialty. The OpenNotes project is demonstrating and evaluating the impact of sharing encounter notes online between patients and their primary care physicians - a form of 'open access'. popHealth

popHealth® is an Open-Source Clinical Quality Measure database and reporting engine with data presented through a web-based interface.  It includes a centralized repository of clinical data.  Data is sent from EHRs via nationally recognized standards such as Consolidated Clinical Data Architecture (C-CDA) Continuity of Care (CCD) Documents or QRDA (Quality Reporting Data Architecture) Cat 1 Documents.
