CQM Aligned Population Health Reporting (CAliPHR)

CQM Aligned Population Health Reporting (CAliPHR) is an open source tool that is designed to calculate electronic clinical quality measures eCQMs at a provider, practice, payment arrangement, and community level to support incentive and value-based payment programs.
