Lifespan Rhode Island Healthcare System's Siemens EHR was apparently causing thousands of electronically-generated prescriptions to become scrambled, as I posted in Nov. 2011 here: http://hcrenewal.blogspot.com/2011/11/lifespan-rhode-island-yet-another.html. Anatomy of a Public Health Open Source Project: HLN's Immunization Calculation Engine (ICE)
An immunization information system (IIS) aggregates immunization information for children (and some adults) living or receiving immunization services in a jurisdiction. One of the core components of an IIS is its immunization evaluation and forecasting system: the computerized algorithm that is used to determine if vaccine doses that were administered to the patient are clinically valid (evaluation) and to project what doses are due now and in the future (forecasting). These algorithms are used to support clinical decision support (CDS) at the point of care and also to help public health agencies understand and manage the immunization status of whole populations.

Data Management for Large-scale COVID-19 Immunization: This is all not as simple as it seems
There is a global race for the development of a vaccine for the SARS-CoV-2 virus that causes COVID-19. Finding a vaccine that works and receives approval is only part of the process. There are a series of other steps that need to be taken so that the vaccine can be delivered. These include the mass production of the vaccine, shipment, administration and record-keeping. This may be even more complex as there may be several vaccines. In this article we review some of these issues with a particular focus on the United States.

Immunization Information System Interoperability… What We’ve Learned Along the Way
The IZ Gateway initiative (IZG) sponsored by the Centers for Disease Control and Prevention (CDC) is up and running. HLN has helped its Immunization Information System (IIS) clients connect to other IIS via the IZ Gateway from planning through testing and go-live. As anticipated, this electronic data exchange has resulted in an increase in record completeness for patient doses received in one jurisdiction, while living in another. Testing between our clients and other IISs, referred to as IIS-IIS pairings, has spurred productive inter-jurisdictional interoperability discussions, while also highlighting nuances in platform configuration. A few of these nuances are provided below.

NASCIO Recognizes Outstanding Achievements in State IT: Finalists Announced for 2017 NASCIO State IT Recognition Awards
The National Association of State Chief Information Officers (NASCIO) has selected 32 finalists across 11 categories for the State IT Recognition Awards. This is the 29th consecutive year NASCIO has honored outstanding information technology achievements in state government through the awards. Projects and initiatives from NASCIO member states, territories, and the District of Columbia were eligible for nomination. NASCIO members served as volunteer judges to review the 100+ submissions, narrowing the nominees down to finalists in each category. From these finalists, a recipient will be announced during an awards dinner at the upcoming NASCIO Annual Conference this October in Austin, Texas...

The Crazy Price of College Textbooks Is Pushing More US Universities to Adopt an “Open-Source” Solution
Seven Rhode Island universities, including Brown and Rhode Island College, will move to open-license textbooks in a bid to save students $5 million over the next five years, the governor announced Tuesday (Sept. 27). The initiative is meant to put a dent in the exorbitant cost of college and, more specifically, college textbooks. Mark Perry, a professor of economics and finance at the University of Michigan Flint, and a writer at the American Enterprise Institute, estimated last year that college textbook prices rose 945% between 1978 and 2014, compared to an overall inflation rate of 262% and a 604% rise in the cost of medical care...

Is Digitizing Healthcare Making It Less Safe?

Silverstein, M.D., a consultant and professor in the Drexel University informatics program who is a leading critic of the claims made for EHR systems and researches the pitfalls of the software and the way it is implemented. He blogs at Health Care Renewal as InformaticsMD. One of the issues he highlights is that there is no systemattic tracking of medical errors associated with functionality or usability issues of EHRs, making it hard to judge whether their net effect has been positive or negative. Yet there are troubling signs, in everything from academic studies to malpractice claims, that the risks of EHRs have been underestimated and the rush to implement these systems may be misguided.
