A recent article in HealthAffairs describes a significant decline in the number of both operational HIEs and HIEs in the planning stage from several years earlier. The authors note continuing barriers to broad-based HIE and a shift to vendor-driven exchange which diminishes the effectiveness of community-based networks. In effect, this translates to a shift away from geographic-based/dominated HIEs to product-dominated HIEs. We have already noted (see The Interoperability of Things) the lack of a national strategy on HIE, and ONC’s Nationwide Interoperability Roadmap barely mentions the concept.

HIMSS19: Open Source Software for Disaster Preparedness and Response
Although not officially listed as a track at the HIMSS19 conference, there are a series of very important presentations on the use of open source software for disaster preparedness and response. This is a critical topic that we have covered extensively in Open Health News. As we detailed in this article, there was a major failure in being able to provide victims of Hurricane Harvey, as well as Hurricane Irma and Hurricane Maria with access to their medical records. Few emergency medical responders could access their records either. The two success stories that came out of the hurricanes were two open source electronic health record (EHR) systems, OpenEMR and the VA's open source VistA EHR.

The Evolving Landscape of Health Information Exchange
The original vision for nationwide health information exchange was a “network of networks” model where local HIEs would interact HIE-to-HIE to form a virtual national network. But notice that many of the new initiatives are essentially solving a different problem: they are enabling point-to-point connections across a wider geography and set of clinical sites. This seems more like a large, single national network rather than leverage of more distributed organizations or implementations. Only time will tell if these private sector initiatives will collaborate, converge or compete. And only time will tell of the limitations of ONC’s ability to influence and provide leadership will creates gaps or provide new opportunities for innovation.

The Strengths and Weaknesses of the HL7 FHIR Messaging Standard
It has been several years since we reviewed the progress of the HL7 FHIR standards adoption rate. Health Level Seven's (HL7) Fast Healthcare Interoperability Resources (FHIR) is an emerging standard that has rapidly captured the mind-share of the Health Information Technology (HIT) standards community. FHIR is a standard that enables healthcare data sharing between systems in a manner that is more easily implemented and more expressive than previous HL7 standards such as HL7 Version 2, 3 and Clinical Document Architecture (CDA). Regardless of the version of HL7 standard used, the purpose of these standards is to send clinical data in messages, whether to a party inside or outside your organization. HL7 devises flexible message formats so the receiver of the message can open it up, know who sent it and why, and break it down into understandable segments and data fields.

Upgrading the PULSE Emergency Response Tool to a Community Edition
Seven years ago the Office of the National Coordinator for Health Information Technology (ONC) began work on the Patient Unified Lookup System for Emergencies (PULSE). PULSE is an effort to create national resilience by improving first responder access to patient electronic health information they need to provide services and ensure public health during disasters, including public health emergencies. Recently, ONC launched a new, code-only technical solution called PULSE-Community Edition (PULSE Community) that builds on the original PULSE code. PULSE Community enables first responders and other response personnel (e.g., epidemiologists, emergency medical services, and health care volunteers) to securely access vital health information they need during emergencies and disasters from connected health care organizations.
