A (Real-World) Tale of 6 Practices and 5 EMRs

We've all been there. Someone in our family (or ourselves) has a medical concern so schedules a primary care visit, gets some images or lab tests, and perhaps learns that surgery is needed. Closing the Referral Problem in Distributed Care Networks

There have been any number of people and companies working tirelessly to solve healthcare's interoperability challenges. Hospitals, EHR software companies, Health Information Exchanges (HIEs), Federal and State governments, payers, and others have developed, tested, and implemented many in-house or vendor-built solutions to try to clear this hurdle. This is great news for patients, but when I am a patient, I also want my doctors and other caregivers to be able to communicate about me when needed. White Paper: Stop the Referral Problem - Building Digital Care Transitions that Reach Your Entire Network

The healthcare industry has crossed a digital chasm-at least in part. Patient records have moved from paper to computer and many transactions, such as e-prescribing and lab orders have been automated, to accelerate workflows, minimize mistakes and reduce costs. But when it comes to sharing patient records, especially beyond the four walls of a hospital, we remain in the dark ages of paper and fax...In this paper, we will discuss our research about how referrals and care transitions are typically conducted; the financial, non-financial, and quality impacts on patient care; and near-term opportunities for leveraging technology to accelerate these processes to benefit provider organizations and to deliver a high-quality, efficient patient experience.
