A Discussion of Medicaid’s $5 Billion/Year IT Infrastructure Transformation

CMS provides funding to the tune of 5 billion dollars per year to support the Medicaid information technology platforms run by the states. In December 2015, CMS issued a final rule, Mechanized Claims Processing and Information Retrieval Systems (90%  Federal/10% State), to assist states to update Medicaid Management Information Systems (MMIS) in over 20 states. These changes will allow states to improve customer service and support the dynamic nature of Medicaid eligibility, enrollment, and delivery systems. Also within this rule was language directing the Medicaid Enterprise towards an open, modular architecture.

Defining An Open Platform for Health IT

It is widely agreed that the future of digital health lies in an “Open Platform”. However, it’s not clear as to exactly what an Open Platform is or how we get there. This blog aims to answer the first question and to provide some guidance on the second. While any given instance of an Open Platform will be a specific implementation of a set of software components owned and operated by a particular organisation (this might be a health and social care organisation or a third party, operating the platform on behalf of a local health and care community), it is most usefully defined by a set of principles rather than the specific details of a particular implementation.

Open Source Governance and the Rise of a New Open Health Movement

It's hard to tell if (or when) new open source foundations will appear and claim a leading role in healthcare. It would be interesting to see one created to scale an existing viable model, such as the one from Oroville Hospital using VistA. Or we could see OSEHRA shifting its focus and expanding its charter beyond just the US government space. Nevertheless, the successful foundation would keep a low barrier to entry for innovators, allowing them to incorporate and scale open source healthcare technologies into commercial products. Time will tell, but what's for certain is that we live in interesting times, and I am looking forward to massive innovation in healthcare in the near future. The time is ripe.
